Registration Information

Registration for the 2013 - 2014 School Year begins January 28th.

Monday, January 28:   6:00 - 8:00 p.m.
Tuesday, January 29:   9:00 - 11:00 a.m.
Wednesday, January 30:  1:00 - 3:00 p.m.

Applications are accepted continuously in the office between 9:00 am and 2:30 pm weekdays.

Please call the office between 9:00 am and 2:30 pm if these times are not convenient or if you have any additional questions.

The following items must be submitted at the time of registration:

Birth Certificate *

Baptismal Certificate

Copy of the child's Immunization Record *

Church Envelope Number (Parishioners)

Copy of the most recent Report Card

Copy of the latest Standardized Test Scores.  (C.T.B.S.)

Age:

Children entering Kindergarten in September 2013 must be 5 years old by December 31, 2013.

Children entering Pre-K4 in September 2013 must be 4 years old by December 31, 2013.

Children entering Pre-K3 Nursery Program in September 2013 must be 3 years old by December 31, 2013.

Effective Oct. 2012:  Tuition & Fees are combined and billed in 10 monthly installments.
The first bill is mailed July 1 and payment is due in August, final payment is due in May.
Students entering OLMC during the school year are billed tuition for the appropriate number of months.

Parishioner Status for Tuition

To be eligible for Parishioner Status for Tuition:

The Family

  1. must be registered with the Rectory.

  2. must contribute at least $250.00 per year to the Parish, using the envelope system on Sunday.

To maintain Parishioner Tuition Rates:

The Children

  1. must attend Mass regularly at OLMC on Saturday or Sunday.

  2. must use their own Parish envelope (the amount contributed is not important.)

  3. attendance will be recorded based on envelopes placed in the collection basket at Mass each week.

  4. if several weeks worth of envelopes are placed in the collection together, they will count as only 1 week.

  5. envelopes turned in to the office during the week will not be considered for attendance/parishioner status.

  6. each child in the family will receive new envelopes in late September.  Attendance recording for tuition status will begin in October.

  7. Reminder letters are sent after a 1-month absence.  If two consecutive months are missed, tuition will revert to Non-Parishioner and an adjustment will be made to your FACTS account.

  8. FACTS has advised us that once this adjustment has been made, the Non-Parishioner status will remain until the following school year.

  9. Please keep the office staff informed of anything that might cause a change in your status.

 

Fees to be paid with application:  

APPLICATION FEE:

$25.00 for each new registrant

REGISTRATION / RE-REGISTRATION FEE:

$125. for each child
PARENTS' GUILD FEE $100.00 for each family - separate check, made payable to "Parents' Guild"

              (Includes one Parents' Guild "lucky 300" ticket)

To be paid using the Church Envelope System:  

PARISHIONER CHURCH SUPPORT:

$250.00 per year

You may download the application packet to fill out prior to coming in, if you wish.  Registration papers will be accepted in the office beginning on the dates listed above.   All six (6) pages must be completed.

The re-registration fee of $125.00 per child is due by January 20th,
payable to "Our Lady of Mt. Carmel School."

Tuition Payments:

Tuition payments must be paid through FACTS Tuition Management Program.  We do not collect any tuition in the school office.

There is an annual billing fee of $45.00 per family.

You will receive only 1 statement which may be paid in full or in 10 installments.  1st payment is due in August, last in May.

You may pay by check, automatic withdrawal, or credit card.

Payments will be due on the 1st or 15th of the month, depending on your selection.

A late fee will be imposed on any account paid after the chosen due date.
